6. Schedule of Contracts (Work-on-Hand)

7. Resumes - Owners & Key Personnel

8. Certificate of Insurance 

9. Three Supplier References

10. Three Owner/Obligee References

Please feel free to provide any additional information you think will be useful such as:

Organization Chart
Business Plan
Description of some of the largest completed jobs
(include contract price, date completed and gross profit etc.)
Letters of Recommendation
Copies of Certifications


* FINANCIAL INFORMATION NEEDED

Please present this information to your independent Certified Public Accountant (CPA). We highly recommend your company provide a year-end Reviewed Financial Statement. Virtually all Surety Companies are requiring this type of Statement. Compiled Financial Statements can result in higher premiums and possibly collateral conditions.

Percentage of Completion Financial Statements only
(Cash Basis Financial Statements are not acceptable)

It is necessary to furnish copies of year-end financial statements for the last three years.  If the most recent year-end financial statement is older than six months, please include an interim financial statement.  The financial statements must be prepared by an independent accounting firm (not in-house CPA) and include the following:

*   CPA Cover or Transmittal Letter
*   Balance Sheet
*   Income Statement
*   Statement of Cash Flows
*   Footnotes
*   Schedules - Contracts Completed and Contracts in Progress

The six-month Interim Financial Statement may be in-house - if it tracks to CPA year-end financial statement, otherwise please have your CPA provide a Compiled Interim Financial Statement.
